Great form. It is a fillable PDF from my desktop, but not from my phone. I like that id doesn’t have excess info that most of us don’t need. Not as confusing or time-consuming as the district one. If parents are the ones doing the homeschooling, then here is the state’s form where you can list out the credentials and all that. Otherwise this form is perfect.
